Jeffrey A. Cabotaje, M.D., F.A.A.P.

Dr. Cabotaje has been in private practice for over 15 years. He completed his undergraduate studies at the University of Notre Dame in South Bend, IN and obtained his medical degree in his home state at West Virginia University School of Medicine. In 1998, Dr. Cabotaje returned to the Midwest to pursue postgraduate training in pediatrics at the University of Illinois at Chicago Medical Center. After his training, he remained at UIC to serve as Pediatric Chief Resident and also worked in the inpatient units and special care nursery at Mercy Hospital and Medical Center in Chicago. In 2002, Dr. Cabotaje joined Pediatrics Unlimited.

In his spare time, he enjoys time with family and friends, traveling, running, photography and design, and if weather permits, relaxation time by Lake Michigan.

Dr. Cabotaje is Board Certified by the American Board of Pediatrics. He is a fellow of the American Academy of Pediatrics and a member of the Illinois State Medical Society.


Elizabeth Wollensak, D.O., F.A.A.P.

Dr. Wollensak was raised in Michigan and moved to the Chicago area in 2000. She attended Concordia University in Wisconsin for her undergraduate degree and then completed medical school at Midwestern University-Chicago College of Osteopathic Medicine in Downers Grove. She then went on to complete her pediatric residency training at Advocate Children’s Hospital in Park Ridge, IL. During her medical training she developed an interest in childhood obesity prevention and adolescent medicine. She participated in the Leaders in Primary Care Track and also initiated a Narrative Medicine program during her residency. 

She lives with her husband, her daughters Poppy and Willa, and her rescued greyhound and enjoys spending time with family, traveling, artwork and outdoor sports.

Dr. Wollensak is Board Certified by the American Board of Pediatrics. She is a fellow of the American Academy of Pediatrics and a member of the American Osteopathic Association.
